Before our appointment:

  • You will receive an electronic Scope of Appointment form that will need to be completed prior to our meeting. By rules of Medicare, before any appointment, we must obtain documentation proving that you gave us permission to talk to you, along with the types of plans you want to discuss.

  • If you haven't already, please fill out our client intake form here.

Also, please remember to have this at the time of the appointment:

  • your Medicare card

  • a voided check if you wish your premium payment to come out of your bank account

How to join the Microsoft Teams virtual meeting?
  1. You should have received an email with a link to our Microsoft Teams meeting. Click on the link.

  2. A separate webpage will open and it will prompt you to either download the app on your computer or join the meeting through your web browser. 

         It's easier and faster to join using your browser.
